A Toolkit to Engage High-Risk Patients in Safe Transitions Across Ambulatory Settings
Transitions of care among ambulatory sites are vulnerable to patient-safety gaps. Patients who transition from one ambulatory care facility clinician to another are especially vulnerable to patient-safety errors, in part due to a lack of effective communication and patient engagement in shared decisionmaking. The Health Research & Educational Trust (HRET) adapted select, evidence-based patient and care partner-centered acute care discharge tools to create a toolkit specifically for use in the ambulatory care setting
